I found online that theme in colorlibs website. Just integrate php mailer and do some makeup to it.

You can use that in your project with Colorlib, footer link! You dont need put my site link on it.

But you need to changes with your informations, in (gmail2.php)

Just download zip and put your site that contact form system, if you give right informations you get mail in your box. System uses gmail but some purpose you need to give some confirm on your google panel.
